Remote Patient Monitoring & Chronic Care Management
Supporting Patients Beyond the Clinic
Compassion First Home Care Services is proud to offer Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) services designed to help patients better manage chronic conditions while remaining safely at home.
These programs allow patients to stay connected with their healthcare team between medical visits while receiving additional support, monitoring, and care coordination.
Our goal is to help patients maintain stability, improve their quality of life, and reduce preventable hospitalizations.
Remote Patient Monitoring (RPM)
Remote Patient Monitoring allows important health measurements to be monitored from home using secure medical devices. These devices automatically transmit readings to a monitoring system so that changes in a patient’s condition can be identified early.
Common monitoring devices include:
- Blood pressure monitors
- Pulse oximeters
- Weight scales for heart failure monitoring
- Blood glucose monitors
How It Works
Our care team reviews patient readings regularly and helps identify concerning trends so that providers can be notified when needed. This proactive approach helps patients receive timely support and intervention before a condition worsens.
Chronic Care Management is designed for patients living with two or more ongoing chronic health conditions that require long-term management. Through this program, our care coordination team works closely with patients and their healthcare providers to ensure that care remains organized, consistent, and effective.
These services provide patients with ongoing support between medical appointments and help improve overall health management.
Chronic Care Management (CCM)
Supporting patients living with multiple ongoing chronic conditions
CCM services may include:
- Monthly patient check-ins
- Medication review and adherence support
- Care coordination with physicians and specialists
- Appointment follow-ups
- Health education and self-management guidance
Conditions That May Benefit
Patients with the following conditions often benefit from monitoring and care coordination:
- Hypertension (high blood pressure)
- Diabetes
- Congestive heart failure
- COPD and chronic lung disease
- Chronic kidney disease
- Multiple chronic conditions
